Your first visit to South Shore Surgical involves a few special steps so that we can get to know you. To understand what to expect please read through this page. You’ll find practical information you need such as what to bring, map and directions to our office, practice hours, payment policies and more. You can also save some time by printing out and completing the patient forms in advance of your appointment.
What to Bring
- Insurance Card
- Any Co-Payment, Co-Insurance or Deductible required by your health insurance
- Referral from your Primary Care Physican
- New Patient Form (See below)
- List of Current Medications with Dosages
- A copy of any diagnostic testing such as CT scans, Ultrasounds, Mammograms that were performed outside of a Community Healthcare System or St. Francis Alliance
What to expect
The first time you visit our office you will be seen by one of our physicians. A complete medical history and physical exam will be performed. The diagnosis will be discussed with you and treatment options will be reviewed. If further testing is required, this will be ordered for you. After your consult, if surgery is indicated our surgical scheduler will meet with you to discuss possible surgical dates and locations. Our team of physicans are on staff at Community Hospital, St. Margaret Hospital (Dyer campus), St. Catherine Hospital and Community Surgery Center. If additional testing or consult from another specialist is required, you may be asked to return for a follow-up appointment in order to determine the best treatment for you.
